Minnesota DHS Announces Approval of State Plan Amendment

The Minnesota Department of Human Services has announced the approval of state plan amendment TN 17-25. This amendment revises the payment methodology for the integrated health partnerships program by implementing a population-based payment that supports care coordination services. The amendment also makes multiple technical and conforming changes.

The Centers for Medicare and Medicaid Services (CMS) approved the amendment on May 2, 2018, with an effective date of January 1, 2018. Read the amendment revising the payment methodology for the integrated health partnerships program (PDF).
