Cancellations
No refunds for cancellation on or after October 31, 2014. Cancellations prior to October 31 will receive a refund minus a $50 processing fee. However, participants who redeem an enrollment code will be charged a fee of $175 regardless whether or not they complete any portion of the self-study.

Event Details

Delivering Medication Therapy Management in the Community is an innovative and interactive training program that explores the pharmacist's role in providing MTM services to patients. Pharmacists have a tremendous opportunity to receive reimbursement for monitoring and improving medication use in patients with complex medication regimens. The program teaches pharmacists the essential skills necessary to become a successful MTM practitioner. This certificate program will enhance pharmacists' clinical expertise in evaluating complicated medication regimens, identifying medication related problems, and making recommendations to patients, caregivers, and health care professionals.

This program has been approved by Minnesota Medicaid, and is a comprehensive, three-part certificate training program. A Certificate of Achievement will be awarded to participants who successfully complete all program components.

THE GOALS OF THIS PRACTICE-BASED PROGRAM:

• Advance public health and patient care through improved medication use.
• Provide training to enhance pharmacists’ ability to effectively provide MTM services.
• Motivate increased numbers of pharmacists to establish MTM services.
• Communicate benchmark practices for providing MTM services.

• The role MTM will play in the future of pharmacy practice.
• A plan for becoming a successful MTM practitioner.
• The essential skills and knowledge necessary for delivering MTM services.
• How to conduct an MTM encounter.
• Practice interviews with patients.

Self-study materials will be available online before the live seminar. Program participants will be provided with details on how to access this information upon receipt and confirmation of registration.

The all day live portion of this program will occur November 14. No make-up dates will be available. Please check your availability for this date before registering, as no refunds will be given for non-attendance at this event.

3. ACPE Universal Activity Number for Post-Seminar Exercise: COMPLETED STUDIES DUE January 30, 2015. No extensions will be permitted.

Following the live seminar, participants must document 12 patient cases, identifying and prioritizing each patient's medication-related problems, develop appropriate interventions, complete appropriate documentation and devise a plan for follow-up.

The American Pharmacists Association is approved by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education, and complies with the criteria for continuing pharmacy education.

Successful completion of the self-study learning component will result in 10 hours of continuing pharmacy education (CPE). An additional 8 hours of CPE will be granted for completing the live component, and an additional 12 hours of CPE for the post cases. All three components must be completed in order to print the certificate of achievement.
