Pharm Nights

Registration is now open for our Southwest Fall Pharmacy Night on December 5 at Jackpot Junction in Morton, MN. MPhA leaders are traveling throughout Minnesota for discussions with local pharmacists. This event is an opportunity to learn about the latest activities underway at MPhA, to learn about Southern Prairie Community Care (a new health plan offering in southwest Minnesota).

Presenters

Mary Fischer, Executive Director, Southern Prairie Community Care

Mary is leading the effort of Southern Prairie Community Care to develop the first Rural Community Care System in Minnesota and the United States. With a focus of implementing Minnesota’s patient-centered health care home concepts, improve coordination between providers and participants, implement data driven care models and engage participants, the system is set to launch in January 2014. Mary has worked in Minnesota’s public health care sector for over 25 years. Her past positions include; Regional Administrator of Avera Medical Group in Marshall, MN and Director of Human Services for Pipestone and Nobles counties in Minnesota as well as Assistant Director of Human Services for the counties of Lincoln, Lyon and Murray, MN. She is a current member of the Minnesota Hospital Association Mental Health Task Force and has held several board positions including; the Minnesota Human Services Redesign Council, MACSSA Children’s Services Committee, Association of Minnesota Counties Futures Planning Group and the National Association of Social Workers. Mary received her Master of Science in Social Work from St. Catherine/University of St. Thomas in St. Paul, MN.

Liz Cinqueonce

liz Elizabeth "Liz" Cinqueonce is Executive Vice President for the Minnesota Pharmacists Association. She previously served as deputy director of the Health Economics Program at the Minnesota Department of Health (MDH) and as manager of E-health and Data Projects at the Minnesota Department of Human Services (DHS). Liz previously worked at MPhA (1997 to 2008), rising through the ranks from membership assistant to special interest group coordinator to director of pharmacy and legislative affairs, and finally to vice president of public affairs. Liz earned her bachelor’s degree in political science from the University of St. Thomas. She resides in Hugo, Minn., with husband Carmelo and daughters Allison, Jayna and Gabriela.

 

Brian J. Isetts, RPh, PhD, BCPS, FAPhA

Brian IsettsBrian Isetts is currently Professor in the Department of Pharmaceutical Care & Health Systems at the University of Minnesota College of Pharmacy.  Dr. Isetts has distinguished himself in the application of comprehensive medication management as a practitioner, researcher and educator.  He is a Board Certified Pharmacotherapy Specialist and has experience working as a pharmaceutical care practitioner, nursing home consultant, and in community and institutional practice.  Brian received his undergraduate degree in pharmacy from the University of Wisconsin, and his PhD in Social and Administrative Pharmacy from the University of Minnesota as a Kellogg Foundation Fellow in the Pharmaceutical Clinical Scientist Program.

Brian Isetts has been extensively involved in developing a new health service in which pharmacists collaborate with physicians and other providers to help patients achieve goals of therapy while identifying and resolving drug therapy problems.  Comprehensive medication management has been demonstrated to improve clinical, economic and humanistic outcomes of care, as well as decrease health expenditures when provided within integrated health care systems.  Brian’s scholarly contributions to evaluating comprehensive medication management span the medical and pharmacy literature including publications in the Archives of Internal Medicine, and in Medical Care.

Dr. Isetts worked 12 years with the Minnesota Legislature to enact the Minnesota Medicaid Medication Therapy Management Care Law.  At the national level, Brian has worked with the American Medical Association to recognize and report pharmacists’ medication therapy management services within official CPT® health reporting and billing nomenclature.  Brian recently completed a two-and-a-half year sabbatical as a health policy fellow in the Centers for Medicare & Medicaid Services working on improvements to the Part D MTM Program and serving in the CMS Innovation Center as a Contracting Officer’s Representative and federal co-lead in measuring and improving medication safety and care transitions across 4,000 U.S. hospitals and health systems in the Partnership for Patients Initiative.

The Minnesota Pharmacists Association is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing education. Successful completion of this knowledge based program qualifies for a maximum of 2 contact hours. These ACPE credits are applicable to recertification requirements for CPhTs.

Participants must sign in, attend the full program, and complete a program evaluation form to receive credit.

Participants must complete an evaluation at the event to obtain credit.

Statements of Credit will not be issued, instead the MPhA will upload your CE electronically through the CPE Monitor System within 60 days from the event. In order to upload the CE, you will need to provide MPhA your NABP e-profile ID and birthdate MMDD.
