All cancellations received by September 17, 2018 will receive a full refund minus a $25 administrative fee. Cancellations received after September 17, 2018 will not receive a refund. All cancellations must be submitted through this online form.
Due to PCI compliance, MPhA will only accept this form via fax or mail. Emails with this completed form attached will not be accepted.
