MPhA’s premier educational events promise to be an action-packed conference bringing together our pharmacy professionals and leaders! The 2018 Annual Learning Networking Event will include the Medication Therapy Management (MTM) Symposium, and breakout sessions in practice management and clincal topics for all pharmacists, student pharmacists, and pharmacy technicians. Educational offerings are designed to provide participants with the latest information to keep abreast of treatment guideline advances, practice innovations and strategies for responding to challenges and opportunities affecting pharmacy practice. Registration

The exhibit booth registration fee includes registration for one representative, as indicated on the next page, who is welcome to attend all meals and social events. We welcome representatives in addition to those included with the exhibit booth fee. If you plan to have additional representatives participate, please note the fee is $150 per representative. Our top-level sponsorships include two company representatives. In order to attend educational programs you will need to register as an attendee.

Assignment of Space

The exhibit application form must be completed with payment to confirm a booth space. MPhA will make final booth assignments based on sponsorship levels, requested booths and registration date.

Exhibit placement for Saturday will be determined based on the number of exhibitors electing to display, sponsorship level, and registration
date.

Exhibit Opportunity: Thursday Exhibitors have the option to exhibit on Friday at no additional charge. If you choose to participate in the Friday portion of the conference, please note this on your registration.

CANCELLATION POLICY: All Sponsor/Exhibitor cancellations received by September 14, 2018 will receive a full refund minus a $50 administrative fee. Cancellations received after September 14, 2018 will not receive a refund.

Any person who attends the MPhA Conference grants permission to MPhA to record his or her visual/audio images, including but not limited to photographs, digital images, voice, sound or video recordings, audio clips, or accompanying written descriptions, and for MPhA to use his or her name and such recorded material without notification for any purpose, including advertisements for future programs and events.

Please do not email forms with credit card information. To protect your data and to comply with PCI standards, the MPhA office will not accept emailed credit card information.
